A76-year-old widowed woman was admitted to the nursing home by her two sons.The client reported loss of pleasure, feeling sick, lack of sleep, bored, empty, helpless, hopelessness, sadness, and expressed death wish. The client did was always withdrawn and did not want to interact with other residents. The client felt that her sons and other relatives had deserted her and she no longer had the desire to live. In the Geriatric Depression 15-Item Scale, the client scored 11 indicating that she is severely depressed. The client had type 2 diabetes and a history of hypertension.

The client will also learn coping skills and be able to cope with the fact that she is in a nursing home and not the home environment (Simning& Simons, 2017).
Implementing a written no-suicide agreement: An agreement will be signed with the client that she will not attempt suicide. The agreement will include actions the client should take whenever she is feeling suicidal.
