Ethical Principles in Action
During their practice, health care professionals are frequently confronted with ethical issues that require them to make decisions based on their moral ideals and convictions. Autonomy, beneficence, nonmaleficence, and justice are the four essential concepts of health care ethics that serve as yardsticks for making fair and ethical decisions. In the realm of health care, these ethical standards are universally acknowledged. When presented with difficult scenarios involving patients, medical practitioners and health-care administrators frequently apply these principles to make judgments.
A Summary of the Case Study
Betsy is a committed pediatric nurse who is known for her compassion and concern for her patients. Alice, her next-door neighbor and friend, lives with her husband and Shirley, their 4-year-old daughter. Alice and her husband practice Christian Science, a religion that emphasizes spiritual healing over medical assistance in most cases. When Betsy pays a visit to Alice and Shirley, she notices Shirley having what appears to be a seizure. The infant becomes unresponsive and goes into a brief stare-down phase, her eyes rolling upward. She appears entirely ignorant of her surroundings during the first 20 seconds of the episode. Alice sits by Shirley’s side and prays during the seizure, but she does nothing else. Betsy is concerned about the little girl’s health and inquires about it with her buddy. Shirley used to have roughly 15–20 such episodes a day until a few months ago, Alice says Betsy, but that number has now dropped to around 12. Shirley’s health has improved as a result of Alice’s prayers and faith. This, however, does not make Betsy feel better about Shirley’s situation. Shirley almost certainly has epilepsy, which if not treated promptly might have serious health consequences. At the very least, she believes Alice should appropriately diagnose Shirley’s symptoms.
Betsy arranges a meeting with Shirley’s parents and Dr. Campbell, the director of her hospital’s neurology department, after realizing the gravity of the situation. She approaches the meeting as an intervention, and she and Dr. Campbell both express worry for Shirley’s health. They emphasize that Shirley’s symptoms improving does not necessarily imply that she will be cured. They understand Alice and her husband’s apprehension because of their religious beliefs, but they emphasize the significance of having Shirley’s symptoms diagnosed. To back up their claim, they give studies that show how seizures can be a sign of disorders like epilepsy, which can have a harmful impact on her cognition and conduct. Shirley’s parents are happy that Betsy and Dr. Campbell are concerned about their daughter’s health, but they are steadfast in their refusal to go against their religious beliefs. They believe that if she prays, she will be healed. Betsy is torn between respecting the parents’ religious views and not intervening in the situation and fulfilling her moral responsibilities as a health care provider.
Analyze the Case Study’s Ethical Issues
Shirley’s behavior is the key element that led to Betsy’s ethical problem in the case study.
Due to their Christian Science beliefs, the parents refused medical help for their daughter. When Betsy witnesses Shirley’s seizures, she believes it is her responsibility to make sure she gets medical help. She decides to discuss the risks of seizures because she respects Shirley’s parents’ religious faith and beliefs. Dr. Campbell is also included in the meeting so that he may provide an objective perspective and help them realize why medical intervention is necessary. Shirley’s parents, on the other hand, are confident that prayer will heal her seizures. Because their behavior could have major health consequences for Shirley, Betsy is concerned that the young girl get proper medical care (Baumrucker, et al., 2017).
Analyzing the Case Study Using the Ethical Decision-Making Model
Moral awareness, moral judgment, and ethical action are three components of the ethical decision-making model that can be used to assess the ethical dilemma raised in the case study. Moral judgment differs from moral awareness in that it recognizes the existence of an ethical issue.
When confronted with a problem, one must choose between the correct and bad acts. Ethical behavior is the result of both moral awareness and moral judgment. Taking the proper steps to fix a problem is what ethical behavior entails. The fact that Betsy realizes the circumstances behind Shirley’s situation reflects her moral awareness. Her moral judgment is shown in her attempt to persuade Shirley’s parents to seek medical care for her daughter because she believed it was the proper thing to do. The action that Betsy takes to overcome the challenge is defined by her ethical behavior. This, in turn, is dependent on her own judgment and the four health-care ethical principles (autonomy, beneficence, nonmaleficence, and justice) that she should follow as a health-care provider.
The Case Study’s Communication Approaches’ Effectiveness
When it comes to patient–physician communication, listening is crucial. Betsy learns the frequency of Shirley’s seizures and the decrease in their incidence through listening to Alice. She also discovers that Alice and her husband attribute their weight loss to their prayers and Christian Science faith. As a result, active listening aids Betsy’s comprehension of the issue.
Betsy understands that obtaining medical aid for Shirley without the approval of Shirley’s parents would be a violation of their right to informed consent and a breach of her professional boundaries as a health care provider. As a result, she resolves to provide them with the knowledge they require in order for them to make an informed decision. She has an open line of contact with Shirley’s parents, explaining how seizures affect their daughter’s health. Shirley’s seizures must be diagnosed as soon as possible, she says. As a result, Betsy effectively explains the situation to Shirley’s parents by respecting their feelings and presenting them with thorough information about the dilemma.
During the conversation with Shirley’s parents, Betsy and Dr. Campbell sympathize with Alice and her husband’s reluctance to seek medical care for their daughter due to their religious beliefs. Dr. Campbell appears to have been enlisted by Betsy in order for him to provide his impartial expert judgment based on his expertise dealing with patients with comparable symptoms. They were, however, unable to persuade Shirley’s parents to get her properly diagnosed.
Although Betsy took a methodical approach to dealing with the problem, it appears to have been ineffectual, as Shirley’s parents maintained their confidence in prayer. However, health care workers can learn from this case study by calmly listening to patients’ difficulties and interacting with them while demonstrating empathy and real care.
Applying Ethical Principles to Resolve the Ethical Dilemma
Autonomy, beneficence, nonmaleficence, and justice are four ethical principles that are frequently used to resolve ethical difficulties in health care. Autonomy entails embracing and comprehending patients’ values, beneficence entails working for the benefit of patients, nonmaleficence entails not harming patients, and justice entails treating patients fairly and without bias.
In this scenario, Betsy’s ethical issue encompasses three of the four major principles of medical ethics. The ethical dilemma in the case study is generated by a conflict between the principles of autonomy and beneficence and nonmaleficence on the one hand, and autonomy and beneficence and nonmaleficence on the other. Shirley’s parents’ liberty is preserved by Betsy’s respect for their religious views and refusal to coerce them into getting the girl diagnosed correctly. She also enlists the help of Dr. Campbell to try to persuade them of the need of receiving an accurate diagnosis.
