Areas to asses for an individual who has been sexually assaulted include the physical assessment, medical assessment, as well as psychological assessment. The person should be assessed for any physical injuries, physical examination including a pelvic examination, as well as the collection of specimens and serological tests for sexually transmitted infections (Park et al., 2018). The medical assessment should include assessment for obstetric and gynecologic conditions, the risk for pregnancy, the risk for HIV, and the provision of the appropriate prophylaxis. The psychological assessment should be assessed through direct observation and examination. People who have been sexually assaulted may have rape-trauma syndrome. Rape trauma syndrome is the immediate duration of psychological trauma that an individual experience after the rape and include disturbances to cognitive-emotional, physical, and interpersonal behavior. Emotional reactions experienced by rape victims include humiliation, embarrassment, anxiety, anger, guilt, fear, mood swings, self-blame, depression, and later suffer from posttraumatic stress disorder (Gravelin et al., 2019). The client will verbalize feelings allied to the trauma (anger, hopelessness, guilt, self-blame, humiliation, embarrassment)

Long Term Goals

The client will incorporate the traumatic experience of rape into the persona, renew important relationships, and develop future meaningful goals
The client will show significant progress in handling stages of grief and express optimism and hope (Park et al., 2018).
