I, (INSERT NAME), verify that I have completed (106) practice hours in association with the goals and objectives for this course. I also verify that all required course approvals are in place from my faculty and practice mentor. During this time, I focused on patient falls in relation to behavioral health.
Patient Falls
Patient falls in behavioral health not only increase the healthcare costs but also lengthens patients’ stay in facilities. They may also trigger lawsuits that can run into millions of dollars’ worth of settlements following patient injury. The Center for Medicare and Medicaid Service ceased, as of 2008, to make compensations for injuries resulting from in-patient stays which lengthened patient admission. When a patient falls, the costs are absorbed by the hospital yet falls are considered preventable events. According to the Joint Commission (2015), fall-related injuries happen in hospitals, serious injuries from falls are among the leading top ten causes of sentinel events. Hence, because of the high stakes, it is up to the healthcare professionals, including nurses, to ensure that healthcare facilities have established fall prevention programs that are effective. Nurses can play their role in reducing fall incidences in in-patient care facilities through verbal counseling. Families of patients can be handed out educational information on how to prevent falls. Additionally, Tzeng& Ying (2015) propose the use of a huddle, which is a short informal meeting that is convened by nurses or other healthcare professionals in a facility, to cover issues on patient safety. The nurses can create a post-clinical review on falls to collect information. The information collected can be used to determine the causes of falls, and determine specific interventions for individual patients. A safety huddle will also allow for all persons working in a unit within a healthcare facility to learn from the fall events. Protocols and standards can also be designed and implemented from the huddle findings and which will farther reduce fall incidences among patients.
