Plan for Educational Offering
1. The participants will discuss and verbalize understanding of the AHA Diet and Lifestyle Recommendations elements, which include a healthy diet, regular physical activity, and tobacco avoidance to reduce obesity. The content is the AHA Diet Lifestyle Recommendations to reduce obesity and the Fitbit app’s use for tracking patients’ calorie intake and activity. The project’s measurable outcome, desired outcome, and timeframe for the intervention. The teaching methods will be a combination of verbal and written materials. I will provide a copy of the AHA Diet Lifestyle Recommendations to reduce obesity and the Fitbit app to track patients’ daily meals/calories and activity. The participants will be able to verbalize and teach back what they have learned.
2. 2. Participants will participate in weekend meetings and verbalize their understanding of their role in implementing the AHA Diet Lifestyle Recommendations to reduce obesity and their role in checking intervention compliance. The content will be AHA Diet Lifestyle Recommendations, which include a healthy diet, regular physical activity, and tobacco avoidance to reduce obesity. Staff roles, intervention documentation, checking the Fitbit tracker, and the corresponding Fitbit app to track participants’ daily meals/calories and activity. I will use verbal and written learning materials with handouts provided. I will give every champion a copy of Diet Lifestyle Recommendations to reduce obesity. The participants will be able to verbalize and teach back what they have learned.
3. 3. Providers will demonstrate how to teach participants about using the Fitbit app to track daily meals/calories and activity and the AHA Diet and Lifestyle Recommendations. The education content will be about Lifestyle changes, which include a healthy diet, regular physical exercise, and tobacco avoidance, and the Fitbit app to track daily meals/calories activities that the patient needs to do to reduce obesity. The information provided will be in a handout and other materials about the actions to reduce weight and prevent obesity; easy to read and to take home for review and guidance. The teach-back method will assess their understanding of the education provided. The patient’s weight loss will be the most critical evaluation outcome.
