The Artifacts of Culture Change is a tool created by the Centers for Medicare and Medicaid Services (CMA) to assess and enhance performance of a long-term care facility. Your course project will provide you with the opportunity to become familiar with this tool. You will create a fictitious Long-term Care (LTC) facility for your project and use this facility as the basis to complete the Artifacts of Culture Change tool. Once you have completed the form, you will create an action plan for your LTC and focus on two areas to improve the facility.

You are required to conduct research on the roles and the requirements for the various parts of this tool. It is highly recommended that you utilize the ECPI Online Library to conduct your research. Be sure to use credible sources for your research.

You must complete all sections of the form from the perspectives of a nursing home management team, to ensure the fields all detail the same things (for example, number of patients non-ambulatory, dietary needs, etc.).

Choose two sections that you believe should be improved upon in this facility. Show how you will improve these sections of the facility using the Artifacts of Culture Change Tool. For example, you might write that your facility has positive outcomes, but the workplace practices may need to be improved. You can decide which areas you want to improve in your LTC facility.

Use these two areas to create an an action plan for the improvements.
